The Diabetes & Metabolism Care Clinic (DMCC) is a specialty clinic within the primary care practice at Lovelace Medical Group.

The Diabetes Self-management Education & Support (DSME) program is with the DMCC. DSME supports informed decision making, self-care behaviors, problem solving, and active collaboration with the care team to improve clinical outcomes, health status, and quality of life.

DSME is skills-based and patient-centered to respond to individuals’ preferences, needs, and values.

DSME improves patients’ knowledge and self-care behavior. This improves clinical outcomes, improved reported quality of life, lower body weight, healthier coping, AND lower health care costs.

The program is centered on supporting:

  • Knowledge
  • Skill Competency
  • Barriers
  • Behavior Change in the content areas of Physical Activity, Nutrition, Medications, Monitoring, Problem Solving, Reducing Risks, Living with Diabetes Health Risks and the Disease Process

Who We See

  • The DMCC sees patients for medical managemen and education any adult, non-pregnant patient with T2 DM

The DSME Program for education sees patients that are

  • Newly diagnosed
  • Established diagnoses without prior (adequate) education with an RD or CDE, Basal/Bolus insulin and carbohydrate counting
  • Pre-Diabetes/IFG/ IGT / Metabolic syndrome
  • Obesity and other ‘at risk’ health conditions for diabetes
  • Unintentional Weight loss & need for weight re-gain

DMSE is appropriate for those with pre-diabetes to receive education and support to develop and maintain behaviors that can prevent or delay the onset of diabetes.

Patients that participate in DSME interventions and follow up use primary and preventative services more often, and use urgent/emergency, acute care hospital services less often. This is a cost savings to the patient and the health care system.

The DMCC DMSE program is a Nationally Accredited program by the American Association of Diabetes Educators.  This means the program meets national standards designed to define quality of the evidence-based education and self-management support

 

Nutrition Therapy

The goals of nutrition therapy intervention are to provide a mutually agreeable care plan to:

  1. Promote and support healthy eating patterns to improve health
    1. Attain individualized goals for blood glucose, blood pressure, and lipids
    2. Achieve and maintain body weight goals
    3. Delay or prevent complications of diabetes
  2. Address individual  nutrition needs based on comprehensive assessment of
    1. Personal and cultural preferences
    2. Willingness and ability to make changes
    3. Barriers to change
  3. Provide positive messaging about food choices based on scientific evidence and to maintain the pleasure of eating
  4. Support meal planning and carbohydrate management with practical tools for day-to-day problem solving
