DAISY Award Nomination Form

Lovelace Health System is proud to partner with the DAISY Foundation in their goal to recognize the outstanding work that nurses do every day. Staff is encouraged to nominate a nurse who exemplifies the characteristics of an outstanding role model. Consider the following qualities when nominating a nurse: collaborates with the health care team, incorporates patient teaching, is a positive role model with a positive attitude, demonstrates excellent customer service, delivers compassionate care, abides by values, is in good standing, willing to mentor/precept others, demonstrates excellent assessment and documentation skills, flexible with staffing issues and frequently goes above and beyond.
